Life and disability claims

Life and disability claims are not eligible for online submission.

To submit an application for disability benefits, an employee’s statement, employer’s statement AND attending physician’s statement are required. To make a claim, please download the applicable claim form(s) here and once completed and signed:

  • Mail to Manitoba Blue Cross at PO Box 1046 Stn Main, Winnipeg, Manitoba R3C 2X7
  • Drop it into the secure 24-hour claim box outside our building at 599 Empress Street, Winnipeg, Manitoba
  • Submit via email*
  • Fax to 204.788.5591

Common questions

Can I get an ID card for my spouse?

Your Manitoba Blue Cross ID card is issued in the name of the member (cardholder) only. When you first became a member, you received two ID cards in the mail and your spouse or dependents can use the extra card as needed. You can also view, print or request additional copies of your ID card in your mybluecross online account (select View or request an ID card under the user account menu). You and your spouse can also show providers your electronic card available on that page or in the Manitoba Blue Cross mobile app. (Download the mobile app from the App Store or Google Play.)

Can I submit a claim for my spouse or dependent(s) if they are covered by another plan?

Yes. You may submit a claim for a spouse or dependent with coverage under another plan through mybluecross® as long as they have submitted the claim under their primary plan first.

Can the Health Spending Account reimbursement be sent to me through direct deposit?

Yes. Visit the Update direct deposit in the mybluecross® account dropdown to set up direct deposit using your account number, transit number, and bank number (located on your personal cheque or in your online banking account details). Banking information will be updated within one business day.
